From tld_master_trainers at mailman.knowledgehub.org.za Tue Sep 10 15:07:57 2019 From: tld_master_trainers at mailman.knowledgehub.org.za (tld_master_trainers at mailman.knowledgehub.org.za) Date: Tue, 10 Sep 2019 15:07:57 +0200 Subject: [TLD_Master_Trainers] Updated TLD and PMTCT training slides Message-ID: Dear colleagues, The following training presentations and case studies have been updated and are available on the Knowledge Hub Master Trainer repository: * Abridged ART guideline and TPT training slides * PMTCT training slides * Nondumiso: Viral load management case study * Melissa: PMTCT case study https://www.knowledgehub.org.za/lms/course/view.php?id=28 These presentations include the following changes: 1. Valproate is safe to use with DTG. 2. The best regimen for a pregnant woman that was previously exposed to ART, but who is not currently on ART, is: a. TLD (if she had a previously suppressed VL), or b. ALD (if she had an elevated VL, or no VL result available whilst previously on ART). 3. Pregnant women who had TPT deferred in pregnancy because her CD4 was > 100 should be reconsidered for TPT at six weeks post-delivery, as 12 weeks is not aligned with any EPI visit. 4. Cervical cancer screening must align with the Cervical cancer policy to screen every three years in women LHIV (if the previous smear was normal). 5. Regarding the risk of NTDs, the risk is lower than previously reported, i.e. 0.1% on EFV vs 0.3% on DTG. TEE is therefore recommended for women wanting to conceive or women pregnant in the first six weeks of pregnancy (while the neural tube is still open). However, we still have to enable her to make an informed choice by: a. Understanding her fertility intentions and pregnancy status b. Providing her with the information on risks (new figures) and benefits c. We should continue to strongly recommend contraception for any women who do not currently want to become pregnant, and give her a choice of contraceptive options. We continue to strengthen integration of family planning into ART services as a priority. Note that the ART and PMTCT national guidelines available on the Knowledge Hub are the current versions approved by the National Department of Health. The training videos from the July provincial training workshops are now live on the Knowledge Hub https://www.knowledgehub.org.za/lms/course/view.php?id=28.
